Fees and Policies

Payment Policy

To help us keep billing simple and consistent, payment is due on the day services are provided. We will direct bill if the option is available and clients are responsible for the balance.

Payment Policy

24-hour notice is required to cancel or reschedule appointments. Missed appointments will be charged full session fee.

Payment options

Credit, Debit, Cash, E transfer, or Direct Billing to Benefit companies. We direct bill to many companies offering direct billing. If direct bill is not available with your company, then payment in full is required and you will be provided with the receipt to submit to your insurance company so you can be reimbursed directly.

Private insurance and health benefit plans vary greatly so it is important to check with your provider to learn about your coverage.  We now offer psychotherapy service coverage provided under "Psychotherapist," "Psychologist," or "Registered Social Worker."

Privacy

Your privacy is very important to us. This policy explains how your personal health information is collected, used, and protected in accordance with the Personal Health Information Protection Act of Ontario and the Standards of Practice of our regulatory colleges. 

 

Collection of personal health information 

We collect personal health information directly from you for the purpose of providing services. This may include contact information, personal and mental health history, relevant background, treatment notes, and information needed to support your safety and care. Only information necessary for treatment and clinical documentation is collected.

Use of information 

Your information is used to provide safe, ethical, and effective Services. This includes assessment, treatment planning, treatment documentation, coordination of care when appropriate, and communication related to appointments. Records are maintained according to Ontario and regulatory college standards.

Your consent is obtained before collecting, using, or disclosing your information. You may withdraw consent at any time except where information must be retained for legal or regulatory purposes. Consent is reviewed during your first session.

Confidentiality and Legal Exceptions

Your information is confidential. It will not be released without your permission except in situations required by law, including:

Concern that you may be at imminent risk of bodily harm or death to yourself

Concern that you may be a risk of significant of bodily harm or death to someone else

Concern that a child or vulnerable person has been harmed or is at risk

A court order or subpoena

Specific mandatory reporting obligations, including harm by a registered professionals

These limits of confidentiality are reviewed with you at the start of therapy.

Storage and Security

Your records are stored in secure, encrypted systems that comply with PHIPA. Access is restricted to myself as your therapist. Records are retained for the period required by Ontario law and the OCSWSSW Standards of Practice, and are securely destroyed when no longer required.​

Electronic Communication

Email and online messages are used only for administrative purposes such as scheduling. They are not used for therapy. While efforts are made to protect privacy, electronic communication carries inherent risks. You may choose your preferred method of contact.

Access to Your Record

You may request access to your clinical record or request corrections to information that you believe is inaccurate. Requests must be made in writing. You will be informed if any part of the record cannot be released due to legal requirements.​

If you have questions about your privacy or how your information is handled, you are welcome to ask at any time. You may also contact the Ontario College of Social Workers and Social Service Workers for more information about your rights.
